成果

• 専門的・学術的観点からの成果: これまで組織的・体系的に研究が行われてこなかった稀少難治性びまん性肺疾患(ヘルマンスキーパドラック症候群合併間質性肺炎,肺胞微石症,難治性気道疾患)について全国的な疫学調査を行い、全国共通の診断基準・重症度分類等を確立した。また、疾患概念が確立している難治性びまん性肺疾患(肺胞蛋白症,特発性間質性肺炎,サルコイドーシス)については、科学的根拠を集積・分析し、エビデンスに基づいた診療ガイドライン等の作成および改訂等を推進し,完了した。
• 臨床的観点からの成果: エビデンスに基づいたIPF新治療戦略として１）慢性増悪期の新治療戦略、２）急性増悪期の血液浄化療法、３）IPF合併肺癌の治療関連急性増悪予防戦略について横断的共同研究を活発に行った。これらの結果をふまえての我が国初のエビデンスに基づいた特発性肺線維症の治療ガイドラインならびに稀少難治性びまん性肺疾患の手引きを刊行した。今後はガイドラインならびに手引きの普及、随時改訂により、難治性びまん性肺疾患の臨床現場における医療の質の向上を図り、国民への研究成果の還元を促進できるものと考えられる
• ガイドライン等の開発: 当該研究の多くは英文論文・ガイドライン・手引き（特発性肺線維症、稀少難治性びまん性肺疾患、サルコイドーシス）として報告・刊行されるに至りその学術的・国際的意義は極めて高い。2018年には、日本呼吸器学会から、我が国初のエビデンスに基づいた特発性肺線維症の治療ガイドラインの英訳版を世界に向けて発信することができた。
• その他行政的観点からの成果: 全国共通の診断基準・重症度分類・臨床個人調査票の改訂等を実施し、新たな指定難病として認定された疾患（肺胞蛋白症、閉塞性細気管支炎、ヘルマンスキーパドラック症候群）も多く、その社会的意義は極めて高い。難治性びまん性肺疾患患者を対象にした患者会を創設したことにより難病患者のQOL改善が期待され、これらにより難病に対する厚生労働行政の課題へ大きく寄与出来るものと考える。
• その他のインパクト: 特発性間質性肺炎診断と治療の手引き改訂並びに特発性肺線維症の治療ガイドラインに関する教育講演が2017年日本呼吸器学会総会で行われた。さらに我が国初の特発性肺線維症の治療ガイドラインとして全国のメデイアセミナーでも公表されるに至った。
