成果

• 専門的・学術的観点からの成果: 症例・対照研究により、食事からのビタミンE摂取、経口避妊薬使用の関与を示す世界初のデータが得られた。また、疫学調査により、我国における本症発生率が100万人あたり2.51人であることを世界で初めて報告した。病態研究では、本研究班で開発したステロイド性骨壊死動物モデルを用い、骨壊死発生の原因として、凝固異常、脂質代謝異常、酸化ストレス、血管攣縮の関与を明らかにした。ゲノム解析においては、本症とCOL2A1遺伝子異常とに有意な関係がないことを世界で初めて明らかにした。
• 臨床的観点からの成果: 本症に対する世界最大規模の人工物置換術の登録監視システムの解析を行った結果、過去17年間に行われた4024関節から臨床情報が得られ、術後脱臼と 臨床的破綻、および危険因子を明らかにした。また、ステロイド性大腿骨頭壊死症の予防研究として、世界に先駆けて全国10施設による共同研究として、全身性 エリテマトーデス患者に対するステロイド治療時に、3剤併用による予防薬を用いる臨床試験を先進医療としてまず2施設において開始した。
• ガイドライン等の開発: 本症の診断基準および臨床調査個人票の改訂を行った（特に鑑別疾患についての充実を図った）。 さらに、重篤副作用疾患別対応マニュアル（日本医薬情報センター）に、特発性大腿骨頭壊死症に関するガイドラインを掲載した。
• その他行政的観点からの成果: わが国における本症の発生率が100万人あたり2.51人であることを世界で初めて報告したことは、医療費負担を行っている本症の経年的変化の解析を可能とし、医療費補助の判断材料ともなる極めて有用なデータと考えられる。また、診断基準において鑑別疾患をより明確にできるようにしたことは、本症に対する医療費補助の観点からも有意義な成果と考えられる。
• その他のインパクト: 本症に対する市民啓発活動の一環として、一般市民向けの難病講演会を開催し、本症の病態、治療についての理解、普及に努めた。さらに、大腿骨頭壊死症に関する国際会議（ARCO）に参加し、日本における研究の現状を報告、意見交換を行った。 また、骨壊死に関する英文の教科書執筆にも多くの研究者が参画した。
